What Happens When You Request Data Deletion?
Upon receiving your request, we will:

Delete Digital Records
We will remove your personal information from:-
Electronic medical records (as permitted by law)

Appointment history and contact records

Retain Required Legal Records (Offline Only)
Please note that, in compliance with applicable medical and legal recordkeeping regulations, Body By Heart is required to retain a copy of your medical record and treatment history offline for a minimum period of time. This includes:-
A full copy of your clinical records

Treatment notes, consent forms, and procedural documentation
✅ These records will be securely archived on encrypted DVD media stored offline in accordance with HIPAA and state medical board regulations.
❌ They will no longer be accessible via our digital systems, portals, or active databases. -
